Heart Palpitations in Perimenopause: When It's Hormones, When It's Your Heart

The patients who bring this one to me are often a little embarrassed by it, because it sounds dramatic and then their heart behaves perfectly the moment they sit down in my office. A woman in her late forties describes lying down at night and feeling her heart suddenly pound, or flutter, or drop a beat and catch itself — a few seconds of her pulse doing something she has never had to think about in her life. It happens most in bed, sometimes with a wave of heat, sometimes out of nowhere. During the day it barely registers. She has read enough to be frightened and enough to be dismissed, and what she wants to know is simple: is this my hormones, or is this my heart?

That is exactly the right question, and the honest answer is that it can be either, occasionally both, and the way to tell them apart is not to guess. Palpitations are one of the more common symptoms of the perimenopausal transition, and they are also one of the few that can, uncommonly, point at something that needs a cardiologist rather than a hormone panel. So before I say a word about the hormonal mechanism, I want to start where I start in the room — with the patterns that mean you skip the hormone conversation and get your heart looked at first.

First, the palpitations that are not a hormone question

Most palpitations in midlife are benign. But a short list of features moves a palpitation out of the "probably hormonal" category and into "get this evaluated promptly," and I would rather you know them cold than reassure you into ignoring one. Treat the following as reasons to be seen by a physician rather than to wait and watch:

  • Palpitations that come on with exertion. A heart that races or pounds when you are climbing stairs, exercising, or exerting yourself — as opposed to when you are resting or lying down — is a different kind of signal and deserves a cardiac look.
  • Fainting or near-fainting. Palpitations that come with passing out, graying out, or the feeling that you are about to, are near the top of the list. Do not file that under stress.
  • Chest pain or real shortness of breath. Palpitations paired with chest pressure or pain, or with breathlessness that is out of proportion to what you are doing, need prompt evaluation.
  • A family history of sudden death or inherited heart disease. If a close relative died suddenly and young, or carries an inherited arrhythmia or cardiomyopathy, your palpitations get taken more seriously by default.
  • Known heart disease. If you already have a diagnosed cardiac condition, new or changed palpitations are a call to your cardiologist, not a symptom to explain away hormonally.
  • A very fast, sustained, or irregular episode that will not stop. A racing heart that goes on and on, or a pulse that feels chaotically irregular rather than an occasional skip, warrants same-day attention — and anything with chest pain, severe breathlessness, or fainting is an emergency-room matter.

None of this is meant to frighten the large majority of women whose palpitations are benign. The point is narrower: these specific pairings are the ones that earn a cardiologist first. If your palpitations have none of them — they come at rest, they pass in seconds, they leave you rattled but otherwise fine — then the hormonal and metabolic conversation that fills the rest of this article is the right one to have.

What a palpitation actually is

A palpitation is not a diagnosis. It is simply the awareness of your own heartbeat — a beat you can suddenly feel when normally you feel nothing at all. That awareness can have very different things behind it, and lumping them together is where confusion starts.

Sometimes the heart is genuinely beating faster, a true acceleration of a normal rhythm, which is what happens during the adrenaline surge of a hot flash or a fright. Sometimes the rhythm throws in an early or extra beat — an ectopic beat, arising a moment ahead of schedule — followed by a slightly longer pause as the rhythm resets, and it is that pause-then-forceful-next-beat that people feel as a thud, a flip, or a skipped beat. These early beats, whether they come from the upper or lower chambers, are extremely common, they occur in hearts that are entirely healthy, and on their own they are generally harmless. And sometimes, less often, a palpitation reflects a true arrhythmia that needs identifying. The felt sensation does not reliably tell you which of these you are having, which is the whole reason a workup exists.

What matters for a perimenopausal woman with an otherwise healthy heart is that the two benign versions — a normal rhythm running fast, and occasional early beats you have become aware of — account for the great majority of what she is feeling.

Why estrogen withdrawal makes the heart easier to feel

Here is the mechanism, kept honest. Estrogen does more than regulate the reproductive system; it has effects on the autonomic nervous system — the involuntary wiring that sets heart rate and vascular tone — and on the blood vessels themselves. Why the benign kind clusters at night

Almost every patient tells me the same thing: it is worse in bed. There is a straightforward reason the harmless flavor of palpitation loves the nighttime, and understanding it takes a lot of the fear out.

When you lie down in a quiet, dark room, two things change at once. Your parasympathetic tone — the calming, vagal side of the nervous system — rises, which slows your baseline heart rate. Against a slower background rhythm, an occasional early beat and the compensatory pause that follows it stand out far more than they would against the faster, busier rhythm of an active day. And the environment itself removes every competing signal: no conversation, no screen, no movement, nothing to occupy your attention except the one organ in your chest. An early beat that was firing all day long, unnoticed, becomes a thud you feel through the mattress. Add a hot flash, which itself clusters at night and drags its adrenaline surge along with it, and the nighttime becomes the perfect stage for a symptom that was there all along. The anxiety overlap, without dismissing anyone

I have to handle this one carefully, because it is where a lot of women get waved off. So the two are genuinely entangled, and the entanglement runs both directions.

It works like this. A hormonally driven palpitation happens; the pounding heart is frightening; the fear triggers its own adrenaline release; that adrenaline drives more palpitations and a spiral of body-scanning, where you lie there hyper-attuned to every beat, which makes you aware of even more of them. The heartbeat is real, the anxiety is real, and each is amplifying the other. What I refuse to do is collapse that into "it's just anxiety," because that framing implies the sensation is imaginary and the woman is overreacting. The palpitations are physically happening. The autonomic reactivity is physically happening. Anxiety is part of the loop, not a verdict that dismisses the rest of it. The right move is to address the whole picture — screen for the treatable physical causes and take the anxiety seriously as its own real thing — rather than pick one and wave off the other.

Caffeine, alcohol, and the everyday provocations

Before I order a single test, I ask what a patient is actually putting into her system, because the highest-yield fix is sometimes the least glamorous one. A number of ordinary exposures either provoke ectopic beats or crank up the sympathetic tone that makes them noticeable:

  • Caffeine, especially the amount that creeps up when you are sleeping badly and reaching for a third coffee to compensate — a pattern perimenopause practically manufactures.
  • Alcohol, which can trigger palpitations directly, and which classically does so after a heavier-than-usual evening; disrupted sleep from alcohol adds a second hit.
  • Nicotine and decongestants such as pseudoephedrine, plus many pre-workout and "energy" supplements that are stimulants in everything but name.
  • Dehydration and poor sleep, both of which lower the threshold for feeling your heartbeat.
  • Over-replacement of thyroid medication — too high a dose pushes you into a mild hyperthyroid state, which brings palpitations with it.

I am not moralizing about coffee or a glass of wine. I am pointing out that these are common, reversible contributors, and that pulling them back for a couple of weeks is both a treatment and a diagnostic test. The workup I actually order

When the red flags are absent and the everyday provocations have been accounted for, I work through a sensible ladder rather than jumping straight to the dramatic tests. The order is deliberate.

A thyroid panel — TSH with free T4 and free T3. An overactive thyroid is a textbook cause of palpitations, and it mimics both perimenopause and anxiety closely enough to be missed for months. A complete blood count and ferritin. Anemia makes the heart beat faster and harder to compensate for reduced oxygen-carrying capacity, and it produces palpitations, breathlessness, and fatigue as a set. In perimenopausal women this is common and under-checked, because the heavier, more erratic bleeding of the transition quietly drains iron stores — a loop I detailed in heavy periods in perimenopause. Ferritin, not just hemoglobin, is the marker that catches it early.

An ECG. A twelve-lead electrocardiogram is a quick baseline snapshot of the heart's rhythm and electrical intervals. It can reveal an ongoing arrhythmia, evidence of a prior problem, or an electrical pattern worth knowing about. Its limitation is that it only sees the rhythm during the few seconds it is recording, so a normal ECG does not rule out an intermittent problem — it simply clears the baseline.

Ambulatory monitoring, when episodes are frequent enough to catch. This is the step that actually answers the question of what your rhythm is doing during a palpitation. A Holter monitor records continuously for a day or two; a longer event or patch monitor is worn for weeks and captures more sporadic episodes. The goal is symptom-rhythm correlation — you press the button or log the moment you feel it, and we see exactly what the heart was doing at that instant. That correlation is what converts "I feel my heart flutter" into a definite answer, and it is why I reach for a monitor when the episodes are recurring but an office ECG keeps coming back normal.

Depending on the picture I may add basic electrolytes and a review of every medication and supplement on the list. This ladder is intentionally boring, and that is the point: it moves from the common, cheap, and reversible toward the more involved, and it usually lands on a benign answer.

What a hormone evaluation adds, and what it does not

Because this is a hormone-focused practice, I want to be plain about where a hormone evaluation genuinely helps with palpitations and where it does not. It helps when the palpitations are clearly part of the vasomotor cluster — arriving with hot flashes, clustering at night, moving in step with the rest of the transition — and when the cardiac and thyroid workup has come back reassuring. What a hormone evaluation does not do is treat your heart.
