Heavy Periods in Perimenopause: What's Expected, What Needs Evaluation

Every month I have some version of the same conversation with women in their forties. A period that used to be predictable has turned into something they plan their lives around — a day they would rather not be far from a bathroom, a night they sleep on a towel, a stretch of feeling wiped out that they have quietly started to accept as their new normal. The question underneath it is almost always the same: is this just perimenopause, or is something actually wrong? Both can be true at the same time, and the useful work is sorting out which part of the change is expected and which part deserves a closer look.

This article is my attempt to give you that framework — why periods often get heavier in this window, how to tell an ordinary shift from bleeding that has crossed a line, the specific patterns I do not want anyone to wait out, and where hormone care genuinely fits versus where a gynecologic workup is the right door.

Why periods change in perimenopause in the first place

To understand why bleeding gets heavier, it helps to know what a normal cycle is doing behind the scenes. In an ovulatory cycle, an egg is released, and the follicle it came from becomes a small progesterone factory for the second half of the month. That progesurge does two things: it matures and organizes the uterine lining, and then, when it withdraws, it triggers a coordinated, contained shed — the predictable period most women grew up with.

Here is where I try to give people something concrete, because "heavy" is subjective and most women have no external reference point — they only know their own periods. The old research definition of heavy menstrual bleeding was more than about eighty milliliters of blood loss per cycle, which is a useless number in real life, because no one is measuring milliliters into a cup. So I use practical markers instead.

The signs that a period has crossed from heavy into worth-evaluating territory:

  • Flooding — bleeding that soaks through a pad or tampon in an hour, especially for several hours in a row.
  • Hourly changes — needing to change protection every hour or more often at your heaviest.
  • Large clots — passing clots bigger than a quarter, or many clots.
  • Overnight protection — waking to change, sleeping on a towel, or doubling up on protection to get through the night.
  • Duration — bleeding that regularly runs longer than about seven days.
  • The way it makes you feel — lightheadedness, breathlessness on the stairs, or the kind of fatigue that is out of proportion to your life.

None of these single-handedly means something is wrong. But they are the language I want you to use when you describe your periods to a provider, because "they're heavy" tells me almost nothing and "I'm changing an overnight pad every hour and passing clots" tells me a great deal. The iron-depletion loop most women underestimate

The part of heavy bleeding that gets missed most often is not the bleeding itself — it is what the bleeding is quietly doing to your iron. Every heavy cycle is a withdrawal from your iron stores, and when those withdrawals outpace what your diet replaces, your ferritin (the marker of stored iron) falls. Here is the trap: ferritin can drop low enough to cause real symptoms long before your standard blood count shows anemia. A woman can have a "normal CBC" and still be running on empty.

But there are specific patterns that I do not want anyone to wait out, because they are the ones that warrant ruling out a problem with the uterine lining. Get these evaluated rather than watched:

  • Any bleeding after menopause. If you have gone twelve consecutive months with no period and then bleed — even a spot, even once — that is postmenopausal bleeding and it needs evaluation. Not next year. It is usually something benign, but it is the presentation we most consistently take seriously.
  • A distinctly new heavy or irregular pattern that begins after 45. New, different, and heavier deserves a look, particularly the further you are into your forties.
  • Bleeding between periods. Spotting or bleeding that shows up outside of your actual period.
  • Bleeding after intercourse. Postcoital bleeding is its own flag.
  • Bleeding that comes with severe pain, dizziness, or signs of significant blood loss.

I want to be clear about what these red flags mean and do not mean. They do not mean you have something serious — the large majority of the time the evaluation is reassuring. What they mean is that the responsible move is to characterize the bleeding rather than assume it is "just perimenopause," because the whole point of evaluation is to catch the uncommon problem early while confirming the common one. The evaluation is how you get to stop worrying, not a reason to start.

What a workup usually involves — and why it is not ours to skip

When bleeding fits one of those patterns, the evaluation is straightforward and worth understanding so it is not intimidating. It generally starts with a pelvic ultrasound to look at the uterus and lining and to identify structural causes like polyps or fibroids. Depending on what that shows and on your risk factors, the next step is often endometrial sampling — a biopsy of the lining, usually done in the office — to directly assess the tissue. Bloodwork typically accompanies this: a complete blood count, ferritin, thyroid studies, and sometimes clotting studies.

That last point deserves a note, because it is a genuine overlap with what I do. Second, hormone therapy is not a fix for structural causes. A large fibroid or a symptomatic polyp is not something progesterone dissolves, and the management of those belongs with gynecology.
