Do Hair Growth Supplements Work? An Evidence-Based Review

There is a specific kind of tired I see in people who have been fighting hair loss on their own for a while, and it usually arrives with a bag. Not a bag of shed hair this time — a bag of bottles. Biotin, a collagen powder, a marine-complex chewable, saw palmetto, a hair-skin-and-nails gummy, maybe a zinc on top. The question is some version of the same thing: I have spent months and a fair amount of money on these, my hair is no better, so what am I actually supposed to take? It is a fair question and it deserves a straight, evidence-graded answer rather than either the influencer pitch or a dismissive wave. The honest version is that a couple of these have a narrow, real use, most have thin evidence, one carries a genuine safety catch that has nothing to do with your hair, and the whole category works best as a supporting act to a workup rather than a replacement for one.

Biotin: real for a deficiency almost nobody has

Biotin is the ingredient with the loudest marketing and the narrowest evidence. Here is the honest shape of it. Biotin is a B vitamin your body needs to build hair, and in a genuine biotin deficiency, hair loss and brittle nails are real symptoms that resolve once the deficiency is corrected. That much is solid.

The problem is that true biotin deficiency is rare. It turns up in specific situations — a genetic enzyme condition, long-term use of certain seizure medications, prolonged consumption of raw egg whites, sometimes after bariatric surgery — not in the average person with thinning hair. And in people who are not deficient, the evidence that extra biotin grows hair is essentially absent. You cannot push a nutrient you already have enough of into producing more hair; the follicle takes what it needs and disregards the surplus. For the vast majority of people swallowing high-dose biotin for their hair, the most likely result is an expensive stream of well-labeled urine. If you are genuinely deficient, correct it. If you are not, biotin is not the answer, however heavily it is sold as one.

The biotin risk that has nothing to do with your hair

This is the part almost no one on the supplement aisle mentions, and it is the reason I raise biotin with every patient who takes it: high-dose biotin can quietly sabotage your blood tests. The FDA has issued a safety warning about exactly this. A great many common lab assays are built on a biotin-based chemistry, and a large amount of biotin in your bloodstream can throw those tests off — in either direction, depending on the test.

Two of the affected tests matter enormously. Biotin can falsely lower a troponin result, the test emergency rooms use to diagnose a heart attack, and the FDA has linked this interference to at least one reported patient death in which a real heart attack was masked by a falsely reassuring number. It can also distort thyroid testing in a pattern that mimics an overactive thyroid — a falsely low TSH paired with falsely high thyroid hormones — which can send someone into a workup for a disease they do not have, or muddy the very thyroid evaluation a person with hair loss most needs to be accurate. The fix is simple and worth knowing: tell every provider that you take biotin, and plan to stop it for a couple of days before any blood work, on your provider's guidance. None of this makes biotin dangerous in some dramatic, everyday way. It means a hair supplement most people do not need can corrupt the results of tests they genuinely do — including the thyroid panel that belongs at the center of a hair-loss workup.

Saw palmetto and the natural-DHT-blocker pitch

Saw palmetto is marketed as a natural alternative to prescription hair-loss medication, and the pitch has a real mechanism behind it, which is what makes it persuasive. The theory is that saw palmetto inhibits the same enzyme — 5-alpha-reductase — that the prescription drug finasteride blocks, lowering the DHT that drives androgenetic thinning. The mechanism is plausible. The evidence is where the story thins out.

The trials behind saw palmetto for hair are small, relatively few, often lower in quality, and modest in what they show. That is a categorically different evidence base from the large, repeated, controlled trials behind the prescription options. I am not telling you saw palmetto does nothing; I am telling you the honest ranking puts it well below the prescription antiandrogens in both effect size and the confidence you can place in it. If someone wants a gentler, lower-potency option and understands they are trading proven effect for that, it is a defensible choice — but it should be made with clear eyes, not with the belief that it equals finasteride in a plant capsule. Collagen: popular, plausible-sounding, thinly studied

Collagen is having a long moment, and the logic sounds airtight: hair is protein, collagen is protein, therefore collagen builds hair. The reality is softer than the marketing. When you swallow collagen, your gut breaks it down into its component amino acids exactly as it would any other protein; the follicle does not receive intact collagen and does not assemble hair out of it directly. What a follicle needs is adequate overall protein and the raw materials that come with a decent diet, and if you are genuinely protein-poor, fixing that will help your hair. A specific collagen product is not a magic version of that.

The direct evidence that collagen supplements regrow or thicken hair is thin. Most of the well-known collagen research looks at skin elasticity and hydration rather than hair, and a good share of it is funded by the companies selling the powder. That does not make collagen useless — it is a fine protein source and generally low-risk — but it means the confident hair-specific claims are running well ahead of the data. If you enjoy it and tolerate it, there is little harm in it. Just do not expect it to do what a real workup and a real plan would do.

The marine-protein studies and who paid for them

The best-studied supplements in this whole category are the marine-protein complexes — the ones built around fish-derived proteins and marketed specifically for hair. In fairness, and unlike most of the shelf, some of these actually have randomized controlled trials showing improved hair counts, which is more than biotin or collagen can claim for hair specifically. I want to give them that credit plainly, because I am asking you to weigh evidence, and weighing evidence means acknowledging it where it exists.

The calibrating detail is who funded the work. The prominent trials behind these products were largely paid for by the manufacturers, and the studies tend to be small. Industry funding does not make a study wrong — plenty of legitimate, well-run research is sponsored by the company with a stake in the outcome, and reflexively dismissing every such trial would be its own kind of sloppiness. But the combination of a clear financial interest and small trial sizes is exactly the situation where you calibrate your confidence rather than either swallowing the claim whole or rejecting it outright. My honest read: the marine complexes are the most evidence-backed option on the supplement aisle, that evidence is real but modest and sponsor-shaped, and they are reasonable to try as an adjunct with tempered expectations — not as the centerpiece of a plan.

Test, don't guess: the deficiencies actually worth ruling out

Here is the reframe I want every patient to leave with. The question is not which supplement should I take. It is: is anything I genuinely need running low, and is anything treatable being missed? Those are answered by testing, not by the supplement aisle, and this is where your money and your months are best spent.

Four things top the list. Iron is first, because low iron stores are one of the most common drivers of shedding I see, especially in menstruating and perimenopausal women — and iron stores can bottom out while a routine complete blood count still reads as perfectly normal. Zinc rounds out the four — a real zinc deficiency can cause shedding, but zinc is also the cautionary tale for guessing, because too much of it causes its own problems and can drive down copper. That is precisely why you test before you supplement rather than adding it on a hunch.

The pattern here is the whole point. A targeted panel tells you what your follicle is actually short on. Supplementing blind does the opposite — it treats deficiencies you may not have while leaving the one you do have unaddressed.

Reasonable adjunct or delay tactic?

So when does a supplement make sense? When it is correcting a deficiency you have actually documented, or when it is a low-risk addition layered on top of a real plan with realistic expectations. A marine complex tried for a few honest months alongside a workup, an iron supplement dosed to a confirmed low ferritin, a vitamin D repletion aimed at a measured level — these are reasonable. There is nothing wrong with a well-chosen adjunct.

The trap is the supplement as a delay tactic. I watch people spend the better part of a year cycling through gummies and powders while the follicle keeps losing ground and the real driver — depleted iron, an underactive or overactive thyroid, a shifting hormonal picture — goes unexamined. Hair biology is slow and partly unforgiving; months spent on the wrong thing are months the follicle does not get back. If a supplement is buying time away from an evaluation you keep postponing, it has stopped being harmless.

It is also worth knowing which non-supplement options actually cleared a higher evidence bar, because that is the fair comparison.
