Botox Aftercare — The Rules That Actually Matter

Botox aftercare instructions vary significantly between practices — some comprehensive, some cursory, and some shaped more by ritual than evidence. Understanding which restrictions actually have a clinical basis helps patients protect their results and avoid unnecessary inconvenience.

The first four hours — what matters

Avoid lying down flat for four hours. This is the most consistently recommended restriction and has clinical basis. Botulinum toxin, before it is fully fixed to the neuromuscular junction, has some theoretical mobility. Lying down in the first few hours after treatment has been associated with diffusion of product away from the intended target in case reports. The risk is low, but the restriction is brief and reasonable.

Avoid strenuous exercise for the remainder of the day. Increased heart rate and blood pressure elevation increase blood flow, which could theoretically redistribute product before it fully binds. More practically, exercise produces sweat and friction that should be avoided at injection sites in the immediate post-treatment period.

Do not rub, massage, or apply pressure to treated areas. Direct pressure can displace product. This includes facial massage, sleeping directly on the face, and applying heavy skincare products with rubbing motions.

The 24-hour window

Avoid alcohol for 24 hours. Alcohol is a vasodilator and a mild anticoagulant. It increases bruising risk, which is relevant in the immediate post-treatment period.

Avoid other facial treatments for 24 hours — facials, microneedling, chemical peels, laser treatments. Treated tissue needs time to recover before additional interventions.

Avoid wearing makeup in the injection area for the rest of the day, minimally, to keep injection points clean and reduce infection risk.

What does not have strong clinical evidence

Facial exercises to "work in" the Botox. No evidence this affects result quality. Botulinum toxin binds to the neuromuscular junction through a fixed biochemical mechanism; facial movement does not accelerate or improve binding.

Avoiding hot showers or saunas for extended periods. The standard recommendation is to avoid extreme heat on the day of treatment — not for days afterward. Brief hot showers the following day are fine.

Sleeping in a specific position for multiple nights. The primary restriction is lying flat in the first four hours. Beyond that, normal sleep position is fine.

When to contact your provider

Significant asymmetry developing one to two weeks after treatment that was not present at two to three days post-treatment. Drooping of the eyelid or brow beyond what was discussed or expected. Any signs of infection at injection sites: increasing redness, warmth, swelling, or discharge.

Mild asymmetry at 24-48 hours is normal — the product is still activating. The full effect develops over two weeks. Evaluation for touch-up is appropriate at two weeks, not earlier.

Maximizing longevity

Neuromodulator results typically last three to four months. With consistent repeat treatment, many patients find results extending to four to five months as the muscle's activity is conditioned to a lower baseline. Avoid active skincare ingredients (retinoids, exfoliating acids, vitamin C) for 24 hours post-treatment. Makeup is generally fine after a few hours, applied gently without pressure. Avoid facials, microdermabrasion, and heat treatments for at least 48 hours.
